ENROLLMENT REQUIRMENTS

1. The child must be between 1.2 – 4.11 years old to be eligible for enrollment;
2. An interview with the parent/s or official guardian of the child
3. Completely filled-up Student Information Form
4. Three (3) pieces 1×1 ID picture of the child;
5. A photocopy of the child’s birth certificate*
6. A photocopy of the child’s vaccination records* or a medical certificate issued and signed by the child’s Pediatrician;
7. A photocopy of past progress reports (eg. narratives, checklists, report cards, etc.) if transferring from a different school; and
8. Filled out Fetcher’s Card Form
   *Show original/CTC copy when submitting requirement record
